Breakthrough IFIs can still occur despite antifungal prophylaxis. In Fatima's case, she had not been taking her full prescribed dose of posaconazole (300 mg qd prescribed, 200 mg qd taken), resulting in sub-optimal prophylaxis and increased risk of breakthrough infection.25
Rapid implementation of diagnostic tests and timely treatment are important. Lack of definitive disease confirmation often causes delays in commencing treatment, increasing the risk of IFI-associated mortality.1,12
