Fatima began to develop a severe headache. Physical examination showed that she was febrile, with diminished respiratory sounds at both lung bases. Her neurological examination was normal initially, but after five hours she became drowsy with left lateral gaze deviation and was transferred to the ICU.
She was then placed on mechanical ventilation.
Central nervous system (CNS) aspergillosis was possible. An intraventricular catheter was placed for external drainage, respiratory samples were collected by bronchoalveolar lavage (BAL) and a diagnostic pleural fluid tap was performed.
